Kane county transition planning council is a network of community stakeholders committed to assisting people with disabilities and their families to make the transition from school to full participation as adults within their communities.
Services
The TPC strives to provide a variety of resources and training for stakeholders in the Kane County Area. The TPC acts as a liaison between education and community service providers. It reviews current trends in transition services and shares information through trainings and community networking.
