History:
The Kane County Transition Planning Council was founded in the late 90's. Initially to meet state legislative directives for the creation of such councils between school districts and state rehabilitation services. The council is composed of educators from Kane county schools and Coop's, transition specialists, Department of Rehabilitation councilor's, vocational coordinators, adult service providers, representatives of higher education, parents and youth with disabilities. For a full member list see the members page under links. The members strive to identify and address needs for transition age youth. A number of area services found their start in the work conducted by this TPC. Some key components of the TPC are networking, collaboration, education/training and the enhancement of services for transition age youth to create smoother and more successful transition outcomes for our area youth.
